This includes working with the service user and their team to create an individualized service plan that includes the person's aspirations and goals; ongoing monitoring of services and completion of service documentation to ensure that the individual's needs are being addressed and regulatory standards met.

Estimated Salary: $20 to $28 per hour based on qualifications.
